I used a SmartMedia/SD adaptor with my ESX-1, this worked well for me. 128mB SD cards are much easier to find than SmartMedia ones...

The same. It was easy to find smart media card and adapter on ebay at least 8 years ago. But now maybe you can find used emx with card and adapter.
Psu was not a big deal for me. Three years ago I moved from the USA to Europe and purchaised a european psu replacement on ebay, there was plenty of them.
