Is your volume knob at 100% while comparing L/R to 1, 2, 3 & 4? I ask because I see the same thing, but for me it is down to having the volume knob at around 50%. L/R is muted by the main volume knob, Outputs 1-4 is going out at 100% regardless.

L-R output jacks are digitally controlled by switching transistors that mute output op-amps of channels according to your
output L-R routing and the sensors are activated by jacks negative body so your actual problem is strictly hardaware !
Either 1/4 female board jack joint is discontinued and broke under violent cable stretching by sound engineer's personal
nor one driving mutting mosfet is defected and I believe you have to visit service center.
