Pro-Tools is infamous for not liking other gear. The best way to use MIDI in Pro-Tools is to use the actual MIDI ins and outs of your Digidesign interface. You may get it working otherwise for a while, but one day it will stop working for no apparent reason.

I've gotten the R3 to work in Pro-Tools 8 (LE) via MIDI (controlling a soft synth) using the USB connection. I'll need to check the settings when I get home tonight.
I'm pretty sure I used the "r3 kbd/knob" setting in the Midi set-up in Pro-tools for the R3.
Then on the Midi track I select the R3 for the input and for the output I point to the instrument track.
On the instrument track is where I insert the soft synth, and for the input I select (and this is where I'll need to check) either the R3 or the Midi track (or maybe it's blank).
The output is typically the stereo out on the hardware.
Let me know if this helps, and I'll check again this evening and post the set-up I have.

Just checked my set-up
On Instrument track:
In the plug-in section, insert the Soft Synth of choice
no input
output is to whatever hardware output you are using (in my case it is stereo 1-2)
On the Midi track:
In is set to R3, KBD/Knob
Out is set to the Soft Synth set up on the Instsrument track above
Hope that helps (btw, make sure the instrument track is stereo).
