Are you doing a quick format instead of a full one ?
Perhaps the usb stick isn't fully erased....??

Personally, I've never had any issues performing the update, but I certainly have seen the situation that Pedro mentioned where some unarchivers(particularly on Mac) will also unpack the actual update ...
