I'm really happy with a little program called GN MIDI that does just about everything you want with your midi or karaoke files. For some time I've been wanting to add Chord Tabs to some Karaoke files to make it easy to play along with them but couldn't find a karaoke text editor that would do the job.
I've now tried this program which is available for free trial (and the full registered version is only $30US). It does exactly what I wanted.

I've found you can download a karaoke file from the internet, open it with this program. Use the inbuilt Guess Chords function then use the inbuilt karaoke editor and add the chords to the displayed lyrics.
eg. [Eb] Its a little bit [Amaj7] funny...
Just thought I'd share this info in case someone is looking to do the same.
The big Cubase etc programs no doubt do all this but hey for $30 I think its a bargain.
All the best,
John

GN Midi has a mid to kar conversion. I've tried it and it works. It assumes the midi had embedded lyrics. So if you have a kar file then re-mix it with your pa1x and it now shows up as a mid file you can open the file with GN MIDI and press the Mid to Kar conversion button in the program it will now show up on your pa1x as kar. Hint - after pressing the conversion button use Save As, then change the bottom box Save as type: to "All Files(*.*)" - (no need to type .kar it does it automatically).
Are you sure Cakewalk can't do this??.
All the best,
John
PS: I've got no experience creating a kar file from the beginning. The program will do this but I can't compare to what you have got. As far as editing or adding in Chord Tabs its very easy. Ctrl K brings you to the editor and you simply click on the words and start typing in your Chord Tab eg [Em]. Some lyric lines are locked but all you do to unlock then is click on the little red lock, change it then re-lock if you wish. It doesn't upset the sync and lyric highlighting on playback works as normal.
