MIDI Foot Controller Suggestions

I'm interested in exploring the possibility of adding a MIDI foot controller to my setup, to cut down on the number of things I have to do with my hands. I'm aware of the major ones (Behringer FCB1010, Roland FC-300, Yamaha MFC-10), and was looking for other suggestions that would meet the following criteria. If you know the names of any other pedals, please suggest them, even if you don't know how they work, and I'll check them out.

Must Have

1) Must have normal MIDI output, not (only) via USB or RJ-45 (computer networking cable, which is what the Line6 foot pedals use to talk to Line6 amps and PODs). I don't always use my laptop, so I can't be tied to routing MIDI through my laptop via USB.

2) Must be able to send control changes (I don't really care about program changes)

3) Must be reasonably small and cheap.

Highly desired

1) Stomp box mode (hit a switch once to turn it on and again to turn it off)

2) Software to program it (though if I have to, I'm comfortable programming it via SysEx)

3) A large number of foot switches for sending CCs (8+)

4) Only two rows of foot switches: this will sit behind the legs of my keyboard stand, so more than two rows will make the back rows quite hard to reach.

5) Along the same lines, a unit that's not very deep.

Optional

1) Expression pedals: I already have two and don't need more from this controller (though I could obviously stop using and/or sell the ones I have)

2) Responds to MIDI program changes: I'll likely use the device only to send out fixed CCs (i.e. the bottom-left switch is always a CC #80 toggle, the next switch over always sends CC #93 momentary), so I don't have to think about which switch does what, but not being tied to this kind of setup forever would also be nice.
