Creative Kontrol Pack Vol. 3 - missing authorization codes?

I bought an X50 a few weeks ago after poking around the 'net and doing some research and reading what people had to say about it in relation to the Yamaha MM6. At first it met my expectations but the more I read in the manual and online, I'd have to say that it has vastly exceeded my expectations.

Anyway, I would like very much to use some of the programs included on the free Creative Kontrol Pack VOl. 3 DVD-ROM that came with it, but I can't find the various authorization codes I need. It's nowhere on the plastic DVD case or on the couple of promo pieces (one for Reason Adapted and one for M1 LE) that came inside, or on the DVD itself. Could it have been stuck to the plastic wrap? I didn't pay much attention to that.....I tore it off and threw it out several trash pickup cycles ago......

How do I go about getting these authorization codes? Should I fill out the form at http://www.korg.com/services/promotions ... coupon.pdf and send it in?
